    No convictions for DUI or DWI within the past 5 years.
  • Boyd Bros.:
    Must not have had an Alcohol offense (DUI, DWI, OUI) in the last 5 years.
  • CalArk:
    DUI, none in last 5 years (only one ever).
  • Carter Express:
    No alcohol related driving offenses in the last 5 years. Alcohol related driving offenses reviewed on an individual basis
  • Celadon:
    No DWI, DUI, careless, or reckless driving convictions in the past 5 years.
  • CRST:
    No DUI, DWI, or open container violations in the last 5 years. No more than 1 DUI in the last 15 years and no more than 2 DUI’s in your lifetime.
  • Crete Carrier:
    No Drug/Alcohol Violations in the last 5 years.
  • CT Transportation:
    No DUI within the last 5 years.
  • CTL Transportation:
    No DUI within the last 5 years.
  • Falcon Transport:
    No DUI within the last 5 years.
  • FirstFleet:
    No DUI/DWI, drug or alcohol related convictions within past 5 years.
  • Foodliner:
    No DUI within the last 5 years.
  • Hogan Transport:
    DUI/DWI has to be at least 5 yrs old.
  • Hornady Transportation:
    Any DUI, DWI, or OWI must be over 5 years old.
  • Hunt Transportation:
    No DUI or DWI convictions or positive drug screens or events in the past five (5) years.
  • Jack Cooper:
    No DWI, DUI, or reckless driving convictions within the last 5 years.
  • Koch Companies:
    DUI/DWI needs to be 5 years old and no more than one offense in a lifetime.
  • Lone Star Transportation:
    No DUI within the past 5 years (only 1 in a life time and none in a CMV).
  • Maverick Transportation:
    No DUI or DWI in the last 5 years.
  • May Trucking:
    No DUI convictions in the past 5 years.
  • Melton Truck Lines:
    No DUI’s, or DWI’s in the last 5 yrs.
  • Midwest Transport:
    No DUI or DWI in the last 5 years.
  • Millis Transfer:
    No DUI or DWI in the last 5 years.
  • Mr. Bult’s Inc.:
    No alcohol or drug related charges within the past 5 years.
  • Navajo Express:
    No DUI/DWI less than 5 years old (No more than one in a lifetime).
  • PAM Transport:
    No DUI/DWI in the last 5 years.
  • Paschall Truck Lines:
    No felonies or DUI’s in the last 5 years.
  • PITT OHIO:
    No serious violations in the last 5 years.
  • Premier Transportation:
    Have not been convicted of any substance or alcohol related offense within the previous 5 years.
  • R & L Carriers:
    No DUI or DWI in last 5 years.
  • Rush Trucking:
    No drug or alcohol related offenses in the last 5 years.
  • Salmon Companies:
    No felonies or DUI’s in the last 5 years.
  • Schneider National:
    No drug or alcohol offenses within 5 years, 1 in the last 7 years, 2 in a lifetime.
  • Southern Refrigerated Transport:
    No criminal convictions (misdemeanors including DUI’s and DWI’s) within the last 5 years. (Includes probation time)
  • Stevens Transport:
    No DWI, DUI in the past 5 years.
  • Super Service:
    No drug or alcohol related offenses in the past 5 years, no more than 1 in a lifetime.
  • Swift Transportation:
    No DWI or DUI convictions within the past 5 years.
  • The Waggoners Trucking:
    No DUI, DWI, drug or alcohol related convictions within the last five (5) years. No history of drug and/or alcohol abuse in connection with the operation of a commercial motor vehicle. Ability to enter Canada may be required.
  • U.S. Xpress:
    Must be 5 years old.
  • XPO Logistics:
    DUI/DWI/BACs convictions must be 5 years old. If the license was suspended, driver is eligible for employment 5 years after their license is reinstated.
